( 1 )This sales price reflects a weighted average of multiple prices ranging from $5.54 to $5.60. Full information regarding the number of shares sold at each price will be provided to the Securities and Exchange Commission staff, the Issuer or any security holder of the Issuer upon request.
( 2 )As previously reported on a Form 4 filed on July 1, 2015, on June 29, 2015, the Reporting Person was granted qualified stock options to purchase up to 83,333 shares of common stock of the Issuer (the "2015 Options"). The 2015 Options were granted pursuant to the Issuer's 2015 Equity Incentive Plan.
( 3 )The 2015 Options were to vest and become exercisable in four equal annual installments with the first installment vesting on June 29, 2016. However, in January 2018, the Reporting Person resigned as Executive Vice President of the Issuer, and the 2015 Options that did not vest as of the date of resignation were terminated.
